  • CDIB, letter of descendancy, or tribal membership card from a federally recognized tribe (living anywhere)

How to Apply

Please call to make an appointment at any Choctaw Nation health clinic for an evaluation or counseling session. Psychiatric services will require a referral from a primary care provider.

Additional Information

Please ask for help with addiction and/or mental health. The Behavioral Health team is available. The following resources are available through the Substance Abuse and Mental Health Services Administration (SAMHSA):
