Medical Records – Health Information Management (HIM)
Patients can obtain a copy of their information by requesting it from the Health Care Center or clinics.

All medical charts are property of Choctaw Nation. All requests must be in writing and contain sufficient information to identify the patient requesting the record. Each patient of the Choctaw Nation Health Services Authority has a unique medical chart which provides a detailed record of health care services provided over the years. When a patient visits one of our clinics for the first time, the registration professional must create a new medical chart. This process usually takes 15 to 30 minutes.
