Diabetes Wellness Center

Designed to treat diabetic patients for symptoms arising from their disease in a safe, comfortable environment with certified diabetes educators throughout the reservation.

Eligibility

  • Certificate of Indian Blood card (CDIB), letter of descendancy, or tribal membership card from a federally recognized tribe
  • Consult from a Choctaw Nation primary care provider

Additional Information

An endocrinologist and mid-level providers are dedicated to providing excellent care in the treatment of diabetes and related illnesses. The facility has an accredited education program through the American Association of Diabetes Educators with certified diabetes educators and registered dieticians. On-site pharmacy education, dental services, diabetic retinal screenings, fitness coaches, and case management completes the care provided to patients.

Certified diabetes educators and RN diabetes educators are available in the outlying clinics for diabetes self-management training education. They work with DWC providers to offer case management services for patients needing additional assistance with glucose control. The Diabetes Wellness Center received the Centers of Excellence Award in 2019 from The Joint Commission and was the first to receive this award in an outpatient setting. The Center received continuation of this award in 2022.

Choctaw Nation has health lifestyle coaches that teach diabetes prevention both on site and virtually.
