Burial Program

The Choctaw Nation has two options available for assistance.


  • Tribal Burial
    • Applicants must notify the Tribal Burial Program at time of death
    • Application must be received within 30 days of passing
    • Copy of front and back of Choctaw CDIB or tribal membership card for deceased
    • Copy of SSN card for deceased
    • Copy of Certified Death Certificate
    • Itemized billing statement for verification of vendor (funeral home) information
    • W-9 Statement from vendor (funeral home)
  • BIA Burial
    • Must reside within Choctaw Nation service area
    • Must be a member of any federally recognized tribe, verified by a tribal membership card and valid CDIB card
    • Indigent, with no other resources available and cannot be used in conjunction with other tribal burial program dollars. The deceased may not have resources (life insurance, veteran's benefits, cash, saving accounts, etc.)
    • Residential verification that the deceased has lived within the Choctaw Nation service area for six months prior to date of death (must supply a utility bill or mail with physical location with listed address as proof of residency).
    • Proof of income for the deceased. Verification documents include, but are not limited to, pay stubs, bank statements, copies of benefit checks, saving accounts, or award letters.
    • Copy of Certified Death Certificate

Additional Information

Payments are issued directly to the funeral home, no reimbursements are allowed. If an invoice is paid in full, program eligibility is voided.
