Protect yourself from COVID-19, influenza (the flu), and RSV (respiratory syncytial virus infection). Care kits are available to all tribal members with a Chahta Achvffa account. These can be useful to prevent serious illness from several diseases that are spread quickly through the winter months. Care kits are available for children, adults, or the family. Kits can only be ordered through the Chahta Achvffa portal. One kit can be ordered per tribal member.

Child Care Kit

  • 2 youth cotton masks
  • 1 box of youth disposable masks
  • 30 count of alcohol antibacterial wipes
  • 1 pulse oximeter
  • 1 forehead infrared thermometer
  • 1 COVID-19 home test kit

Adult Care Kit

  • 2 adult cotton masks
  • 1 box of adult disposable masks
  • 30 count of alcohol antibacterial wipes
  • 1 pulse oximeter
  • 1 forehead infrared thermometer

Family Care Kit

  • 2 adult cotton masks

FAQs

This program is available to Choctaw tribal members and offers a variety of goods such as masks, thermometers and antibacterial wipes to help them counter COVID-19. These are available in packages that are meant for either children, adults or families.

The program is only open to Choctaw tribal members. They must live in the United States of America as we are unable to ship overseas at this time.

Applications for this program are accepted online through Chahta Achvffa beginning July 15.

If you go into your Chahta Achvffa and log in, there should be a scrollable window near the top of your page labeled Application Status. This should log the various things you have applied for and the dates you applied. If you have applied for other services, you may need to go to former pages, but it will have the date you applied for a kit and the type of kit you applied for.

Important Information About the Application Process

Kits should be sent out quickly once they are requested. However, due to logistics and delivery time, please allow up to 4 to 6 weeks from the time of request for the delivery.

The application should be processed and the kit received within 4-6 weeks of the initial submission.

Yes. Once an application for a kit has been submitted, an email should be sent to the recipient within the next few hours. While it is not required, it is suggested that the recipient keep the email for their records until their kit is received.

The COVID Care Kit will be mailed to the address the tribal member provides. Please be sure to provide an address that is easily accessible so that the kit can be retrieved.

No additional documents are required for this program.

Any dependent may request a COVID Care Kit as long as they are a Choctaw tribal member.
