A Community Health Nurse will provide vaccinations for patients ages 11 and older. Available vaccines include COVID-19, flu, hepatitis A and B, HPV, meningitis, pneumonia, Tdap, RSV and shingles.
Open to the public.
Requirements
- New patients should bring a birth certificate (if under 18), insurance card, driver’s license (parent/guardian or patients 18+), Social Security card and CDIB card if applicable.
- Insurance will be billed for non-Native patients.
