What types of cards are available?
- Tribal Membership Verification Card – This card verifies and commemorates tribal membership. It does NOT serve as a federally recognized form of ID.
- Tribal Membership Identification Card (ID) – The ID card verifies and commemorates tribal membership and serves as a federally recognized identification card. The ID card contains an expiration date related only to the ID function, but your tribal membership does not expire.As a federally recognized ID (not a REAL ID), your card can be used for purposes such as voting, flying domestically with TSA, and other situations requiring official identification.
- Travel Advisory: We recommend that individuals using the CNO Tribal Membership ID for travel:
- Ask the TSA agent to look up the ID under the T’s for “The” Choctaw Nation.
- Carry another form of ID as a backup.
- It’s important to note that TSA agents have the authority to question or reject IDs they cannot validate under Homeland Security standards.
How do I apply for a new or replacement card?
Complete an Amendment Application available online or upon request.
What are the requirements for obtaining a card?
You will need to complete an Amendment Application, available online or upon request.
For the ID Card application only, submit:
- Passport-style photo with white background, forward-facing, chest and up photo with no glasses or hat.
- A secondary valid, unexpired form of identification, such as:
- Tribal Membership ID
- State-issued ID or Driver’s License
- U.S. Military ID
- U.S. or Foreign Passport (book or card)
- Trusted Traveler ID
