Request information from fellow web visitors
Please fill out the following form as completely as possible. This form is for ancestry research on the website only, it does not go to the Tribal Membership or CDIB office.

If you have a question about tribal membership, need an application, or need to know what to do to begin your paperwork for a CDIB, you must contact the Tribal Membership office at 800.522.6170.

Please use correct punctuation and spelling in your submissions. Do not type in all caps. If they are submitted in all caps and there is no punctuation they will be denied.

Only submit information that relates to your ancestry research. Please do not include information that is derogatory to others or information that may be considered confidential to others.

The listing will NOT be immediately viewable.
Some Suggestions
List one person per form -- but be sure to include plenty of extra information in the comment, such as 'sister of ...' or 'married to ...'
Use the Alternate Last Name for married names, maiden names, or alternate spellings. Fill out another form for the spouse, if you are searching for information on both.
Include you name if you want to, but don't give anyone's Social Security or Roll numbers -- such information will cause the request to not be approved.
